Our records show you previously participated in our WeCare COVID-19 weekly testing program. Due to a change in federal regulations and funding, you must submit your insurance information in order to continue participating in the testing program. Our testing vendor, One Love Laboratories, will bill your insurance provider to cover the cost of the test. There should be no direct cost for you.
Please complete and return this new consent form as soon as possible, preferably by Friday, April 22, 2022. Late consent forms will be accepted and you will be eligible for testing immediately. We will resume our voluntary WeCare COVID-19 testing program on Monday, April 25.
Testing will now be recommended but optional for student-athletes.
